Allergy Testing

Skin Prick Test (SPT)

Skin testing is usually done at a medical professional's office with consent after explaining the procedure. Typically, this test takes about 60 to 90 minutes. A positive reaction to allergy test often looks like a mosquito bite. A skin prick test, also called a puncture or scratch test, checks for immediate allergic reactions to about 50 different substances at once. This test is usually done to identify allergies to House Dust Mites, pollen, mold, pet dander, insect allergen and foods. It is usually done on the forearm or on the upper back. Allergy skin tests aren't painful. This type of testing uses needles, called lancets, that barely penetrate the skin's surface. You won't bleed or feel more than mild, momentary discomfort. Even children usually do not cry. After cleaning the test site with alcohol small marks are drawn on your skin and a drop of allergen extract is applied next to each mark. A lancet is then used to prick the extracts into the skin's surface. Test results will need to be interpreted carefully to avoid a false allergy diagnosis.
About 10-20 minutes after the skin pricks, your skin is watched for signs of allergic reactions. If you are allergic to one of the substances tested, you will develop a raised, red, itchy bump, called a wheal. The size of the bump is measured and results are recorded. Next, the mark on your skin is removed with spirit or by washing with soap and water.

Patch Test

A patch test is a skin test used to find the cause of a possible allergic reaction on the skin. This reaction is called allergic contact dermatitis. Contact dermatitis is a reaction to something that came into contact with the skin eg nickel from imitation jewellery or watch every time you wear it.

Blood Test

Skin Prick Test is gold standard and usually solves the issue. But in specific conditions Specific IgE (sIgE) blood testing may be required. Most of the easily available tests in the market have poor sensitivity and only leads patients in confusion. Such tests where multiple food items are shown to be positive lead to unnecessary avoidance of essential proteins & vitamins from diet.

PFT (Pulmonary Function Test)

They include Spirometry in Adults or Lung Oscillometry in small children which guides us in diagnosing and managing Respiratory Allergy which presents as Asthma or Chronic Cough or repeated wheezing in small children.

Drug Allergy Testing

For most of the drugs there is no standardised blood test available. Most of the freely available blood tests in open market are misleading. Drug allergy is evaluated by detailed history and specific provocation test using the same offending drug (Medicine) in a controlled setup where emergencies can be handled.

Oral Food Challenge Test

Is a gold standard test for definitive diagnosis of particular food allergy. It is also done in controlled set up as it can provoke serious reaction. Skin Prick Test and blood test by Immunocap method or CRD test can only give indirect evidence of sensitivity to that particular food antigen. An expert in the field of allergy only can guide you regarding avoiding particular food.

ASST (Autologous Serum Skin Test)

Sometimes required for evaluation of Chronic Urticaria due to autoimmune cause. Here patient’s own serum separated from blood is injected just below the upper layer of skin and its reaction is noted.

Intracutaneous (Intradermal) Test

Procedure: A small amount of allergen is injected just under the skin.

Response: More sensitive than the skin prick test and used when the SPT results are negative but an allergy is still suspected.

Common Uses: Often used for insect venom or penicillin allergies.
